Annual School-Based Health Screening

Starting next week, the School Health Service will visit our school to conduct the annual health screening. The class received an information leaflet on School Health service today. The first 2 sheets of the booklet contain medical form (GREEN and PURPLE) for the parents to complete with regards to your child's medical history and allergy information. Kindly complete ALL sections of these 2 coloured pages and return to 陈老师, together with your child's health booklet this Thursday.
